State phase/surface validated and files changed.

## Commands run

List tests, build, browser/e2e, provider adapter checks, security checks, and any skipped command with reason.

## Evidence

Link phase proof files, logs, screenshots, traces, runtime artifacts, and `.buildprint/evidence/evidence-ledger.jsonl` rows. Packaged `05-evidence/evidence-ledger.jsonl` is seed-only.

## Claim status

Record what is proven, what remains `PROOF_REQUIRED`, and whether blockers are continuation blockers or qualification blockers.
